Middle School Athletics
For your student to participate in school sports, the following needs to be completed:
A Current Physical
- A doctor must complete this within this school year
Completed Registration Form
Paid Athletic Fee
- The fee is $125 per sport
- Pay online here
Uniform Agreement Form
- Form will be given to students during the first week of practices
- Or download it here
Important Deadlines
- Students must have their authorization to play form and payment completed before the start of their first practice.
- Students must have their physical complete before their first game.
- Failure to complete these items will result in the inability to play.
